USA NON-ATHLETE REGISTRATION FORM 2011
Please note -USA Swimming forms are now fill-in forms. These can be completed online then emailed to Ben as an attachment. If you prefer, you can print out, complete and hand deliver to Ben. A check made out to "YMCA Cape Cod" must accompany the registration form. Athlete forms should be submitted to the Y and Non-Athlete form should be submitted to New England Swimming.
